Context

Add a few sentences about the underlying context behind the system or process being investigated. This section can also add any context on what the ongoing projects were for the associated system.

Summary

Think of this section as the TLDR of the investigation doc. The summary should cover:

• What was the issue?
• How was the issue flagged?
• Briefly summarize the root cause and resolution?

Detection

How was the issue detected (eg: Slack thread, email, PagerDuty, etc)? What is the priority of the issue being flagged? (eg: an issue flagged within a new user signup form on a website would be critical because that could negatively impact user experience and user count)

Investigation Process

This section is very free-form. The investigation process should detail information such as:

• What were the factors being checked? Attach queries, code snippets, any ad-hoc analysis, etc
• What were the conclusions from each of the factors being checked

Try to keep this section fairly readable. The reader should be able to follow the train of thought

Resolution

Detail the steps taken to mitigate/resolve the issue. Mention what were the immediate resolution steps and any next steps/long term action items that follow
